After discharge from the hospital, transplant patients are scheduled for office visits. follow-up visits are scheduled for all autologous and allogeneic patients to evaluate their clinical status and gather essential information for the data manager. In addition, all adults who have had an allogeneic or autologous transplant return to the transplant center for a comprehensive evaluation at 30, 100, 180 days, one year, and yearly after the transplant. Depending on clinical indications, follow-up evaluations at subsequent intervals may be arranged. These evaluations focus on hematologic and immunologic functions, assessment of the original disease, and thorough screening for any late transplant complications. A detailed summary of these findings and recommendations is documented by the transplant physician. Patients are seen by a dedicated Social Work who helps to identify patient and/or family psychosocial stressors and/or dynamics that would interfere with the post-transplant recovery.
